Why do we collect this information?
The WBDC is a non-profit and we never charge for one on one advising sessions. Part of the way we keep our costs down is by collecting demographic information about the people we serve, and sharing this with our funders. The intake form will ask you some questions about you and your business. No public or commercial use of your name, address, or business identity will be made without your prior approval.
I don’t remember certain information (such as my FEIN number, DUNS number, prior year revenues, etc.). What should I do?
We ask you to complete the form to the best of your knowledge. If it’s not a required question (marked with a *) and you’re not sure of the answer, you can skip it. For FEIN number, it is permissible to enter 000000000.
If I stop completing the form midway, can I come back to it?
The registration takes two steps: first you create a profile with your name and email, and then you see the full form. Once you complete step 1, you will get an email with a unique URL, which you can use to return to the form later. The full process should take no more than 10 minutes to complete.
My business partner and I are both coming in for an appointment. Do we both have to fill this out?
We need everyone who comes in to complete the personal information. However, only one person per business entity needs to enter the business information. Your business partner can skip the Add Business popup.
I’m WBENC-certified. Don’t you already have this information?
The WBDC is a regional partner organization for WBENC, but our business advising is funded separately. Please complete the full form to the best of your ability.
How do I know if my form went through?
When you click Finish and Submit, the page will reload with your completed answers and a thank you message. You will also get an email confirming your registration. Be sure to check your spam folder.
The form is not properly recording my selections.
